Fractional CO₂ Laser in Hyderabad – Powerful but Needs Respect

Fractional CO₂ laser is one of the most powerful tools I have for acne scars and skin rejuvenation. Because it is powerful, it must be used with respect—especially on Indian skin, where the risk of pigmentation is real if we are careless.

In the right hands, it can significantly improve texture, scars and fine lines. In the wrong hands, it can cause burns and darkening.

How Does Fractional CO₂ Laser Work?

A fractional CO₂ laser creates columns of controlled thermal injury in a fraction (grid) of the skin, leaving surrounding areas intact.

This:

  • Vaporises tiny columns of tissue.
  • Stimulates collagen remodelling in deeper layers.
  • Over time, leads to smoother texture, softened scars and tighter skin.

Because only a fraction of the skin is treated in each pass, healing is quicker compared to older full-field ablative lasers.

Conditions I Treat with Fractional CO₂

I usually consider fractional CO₂ when:

  • Acne scars are moderate to severe, especially boxcar and some rolling scars.
  • There is significant texture damage or enlarged pores.
  • Fine lines and mild laxity are present on face, under-eye area (with caution) or around the mouth.
  • Selected scars on other body parts need resurfacing.

I almost never use it as a standalone quick fix. It is usually part of a composite plan that includes microneedling, subcision, peels and good skincare.

What to Expect Before, During and After

Before:

  • I assess your skin type, history of pigmentation, keloids, medications and sun exposure.
  • We often prepare the skin with topical creams and strict sunscreen for a few weeks.

During:

  • Numbing cream for 45–60 minutes.
  • Eye protection and careful parameter setting based on your skin and indication.
  • Multiple passes over the area with cooling intervals.

Immediately After:

  • Redness, warmth and a sunburn-like sensation.
  • Swelling in some areas for 1–3 days.
  • Bronzed/"grid-like" pattern and tiny scabs that peel over 5–7 days.

I give very precise post-care instructions: gentle cleansing, healing creams, sun avoidance and strict sunscreen.

Myth-Busting: Fractional CO₂ Laser

Myth 1: "Fractional CO₂ can remove all scars in 1–2 sittings."

Reality: Most patients need multiple sessions (often 3–5 or more) with intervals of at least 4–8 weeks, and even then we talk about improvement, not erasing.

Myth 2: "Stronger settings mean faster, better results."

Reality: Over-aggressive settings on Indian skin can lead to post-inflammatory hyperpigmentation, prolonged redness, or rarely, scarring. I prefer conservative, staged treatments, especially for first-timers.

Myth 3: "Once I do fractional CO₂, I never have to care for my skin again."

Reality: Laser is a tool, not magic. Long-term results depend on sun protection, healthy lifestyle, and sometimes maintenance treatments.

Fractional CO₂ vs Microneedling – How I Decide

  • For mild to moderate scars or early ageing, microneedling (with or without PRP) is often enough, with less downtime.
  • For deeper textural damage, fractional CO₂ provides stronger remodelling.
  • In many acne scar patients, I alternate or sequence microneedling and fractional CO₂ over months for optimal results.

The choice is always customised; I explain all options and we decide together.

My Practice Philosophy with Energy-Based Devices

Because I work across a hospital (CARE Hitech City) and a dedicated skin clinic (Tatva, Moosapet), I see both very simple and very complex skin cases. For fractional CO₂ and other lasers, my guiding principles are:

  • Never choose the machine over the patient—not everyone is an ideal candidate.
  • Prepare and protect Indian skin meticulously to avoid complications.
  • Use the lowest effective settings that give results over time rather than chasing instant transformation.
